Student Survey

Teacher Survey  Parent Survey - English
Parent Survey - Spanish

At the end of each year, 21st CCLC programs must survey participating students and their school-day teacher and adult family member. This is a requirement of the funds that support the 21st CCLC program.

You are being asked to provide information about your perception and impacts of the 21st CCLC program on student academic and personal development. We assure you that there are no ‘right’ or ‘wrong’ answers and no identifying information will link you to the survey. The information obtained will assist the 21st CCLC program in making improvements to better serve its students and families in the future.

Surveys must be completed by Friday, May 17, 2019. The survey links for electronic surveys will be closed after this date. In the survey, you will need to select our program from the drop down:

231. Indian River CSD (C14-S.T.R.E.A.M.): Sebastian Elementary
232. Indian River CSD (C14-S.T.R.E.A.M.): Treasure Coast Elementary
233. Indian River CSD (C15 - ECO STREAM Pelican Island): Pelican Island Elementary

Adult family members may choose to complete the survey in paper format and mail it directly to the survey manager. If you would like to use the paper version, please contact the program coordinator, Susan Thigpen@indianriverschools.org.

21cclc img01Florida's 21st Century Community Learning Centers (21st CCLC) program is a key component of the "No Child Left Behind Act" and is an opportunity for students and their families to continue to learn new skills and discover new abilities after the school day has ended. The focus of this program is to provide expanded academic enrichment opportunities for children attending Title 1 or Title 1 eligible schools. The 21st CCLC grant is authorized under Title IV, Part B, of the Elementary and Secondary Education Act (ESEA), as amended by the No Child Left Behind Act of 2001.
